The service takes a reablement-first approach, focusing on prevention, recovery and reducing reliance on long-term care through short-term, structured interventions.As an Occupational Therapist within this service, you will play a pivotal role in delivering strengths-based, person-centred reablement, helping residents rebuild skills, confidence and control over their daily lives.Key ResponsibilitiesWorking closely with reablement providers, Senior Practitioners and Occupational Therapy Assistants, you will:Complete comprehensive OT assessments within the communityIdentify personalised outcomes and reablement goals with service usersDevelop and issue Reablement Therapy Plans at the point of referralMonitor and review progress throughout the reablement periodProvide professional guidance and consultation to reablement providersApply a strengths-based, preventative approach to all interventionsSupport improved flow through hospital ..... full job details .....
