The successful Occupational Therapist will be based within the Home First South Team, with an office based at Audlett Drive, Abingdon. As a Home First Occupational Therapist, you will:  Assess individuals in their own homes following discharge from hospital. Support safe and timely hospital discharge through the Discharge to Assess pathway. Complete functional assessments and activity analysis. Recommend and prescribe equipment, assistive technology and minor adaptations. Undertake moving and handling assessments and reviews. Work collaboratively with multidisciplinary professionals to develop personalised support plans. Promote independence and reduce reliance on ongoing care, where appropriate. Support individuals, families and carers through periods of change and recovery. Maintain accurate and timely case records.
