As a Nurse Case Manager, your responsibilities will include:

• Overseeing the implementation and management of complex care packages for children.
  
  
• Conducting new client assessments and leading team meetings.
  
  
• Performing medication and clinical audits for compliance.
  
  
• Providing clinical supervision and competency sign-off for nurses.
  
  
• Liaising with the multidisciplinary team for coordinated care.
  
  
• Offering standard and emergency support to regional care functions.
  
  
• Supporting service delivery in line with person-centred care plans.
  
  
• Ensuring care staff are trained in personal care and specialist equipment use.

The ideal candidate for the Nurse Case Manager role will have:

• Registered Nurse (Paediatrics) or Midwife with valid NMC registration.
  
  
• Extensive experience in paediatric complex care including tracheostomy, ventilation, spinal injuries, and brain injuries.
  
  
• Community experience working with both paediatrics and adults.
