The service provides step-down accommodation for individuals moving on from prison or long-term hospital stays , many of whom have a history of substance misuse and complex needs. Key Responsibilities Supporting residents to develop independent living skills, including cooking, shopping, budgeting and household tasks . Supporting attendance at health, probation and external agency appointments . Working in line with individual care plans and risk assessments . Supporting community reintegration and encouraging independence. Managing and de-escalating challenging behaviour in a calm and professional manner. Maintaining accurate records and reporting concerns appropriately. Working closely with colleagues and external professionals to provide consistent support.
