We work in partnership with Sussex Partnership NHS Foundation Trust and GPs, providing integrated support pathways for clients, in particular clients who face more complex mental health situations. As a Mental Health Support Coordinator you''ll be co-located at Foundry Healthcare Lewes and work in partnership with a multidisciplinary team to identify and provide targeted mental health and holistic support. You''ll support clients, from completing initial assessment through to facilitating access to other mental health services. A key part of the role will be to deliver short term recovery-focused sessions with clients, co-creating support plans and goals, and using coaching and motivational interviewing to explore barriers.
