This is a fast-paced role supporting patient flow from psychiatric inpatient units, reducing delayed discharges and ensuring adults are safely transitioned into the community with the appropriate support in place. The Role As a Mental Health Social Worker, you will: Support the timely and safe discharge of adults from psychiatric inpatient settings. Complete strengths-based assessments and discharge planning. Coordinate move-on arrangements from hospital discharge and step-down placements. Work collaboratively with ward staff, NHS colleagues, CMHTs and partner agencies to facilitate safe discharges. Manage complex mental health cases within a fast-paced environment. Liaise with service users, carers and families to ensure person-centred outcomes.
