We have an exciting opportunity for a Long Term Conditions Nurse to join our planned care team at HMP Eastwood Park,

located near Bristol . As a Long Term Conditions Nurse, your responsibilities will include:Ensuring comprehensive handovers when patients transfer between wings or care settings.Identifying immediate health needs and coordinating external services or prescriptions.Facilitating collaboration between agencies to deliver complex care packages, working closely with in-house healthcare services and prison departments such as Pharmacy, Education, Offender Management, Substance Misuse, and Physical Education.Addressing improvement recommendations from CQC and HMPP inspections proactively.Providing continuity of care across health and social care agencies, improving communication between courts, custody, and community services.Enhancing MDT working across the prison and healthcare teams.If you''re interested in career development, we offer a wide range of internal and external opportunities for skill and experience growth.About Us

Registered Nurse - General with relevant NMC registrationAdvanced Nurse Prescriber/PractitionerExperience or knowledge in Diabetes or Sexual Health is desirableDegree-level educationKnowledge of health and social care models within the criminal justice system and relevant initiativesUnderstanding of Evidence-Based Practice and Clinical GovernanceExperience working with individuals with complex health and social care needsExperience in hospital, community, or prison environments is advantageous but not essential.Job Responsibilities
