Band 6 Occupational Therapist - Transfer of Care Team Location: Bedford Start Date: ASAP Contract Length: 3 Months (initially) Hours: Full-Time (37.5 hours per week) Band: 6 - £27 umbrella equivalanet Vacancy Reference: An NHS Trust in Bedfordshire is currently seeking an experienced Band 6 Occupational Therapist to support the Transfer of Care Team within the North Bedfordshire Therapy Service on a locum basis. This role offers an excellent opportunity for an experienced Occupational Therapist to support safe and timely discharge from hospital, while also contributing to admission avoidance and improving patient flow across both acute and community services. Role Overview The successful candidate will work as part of the Transfer of Care Team, supporting patients across acute wards, community services, and rehabilitation pathways. The primary focus of the role is facilitating safe discharge through Pathway 1, ensuring patients are able to return home with appropriate therapy support and community services in place. The post holder will also support urgent care response initiatives aimed at preventing unnecessary hospital admissions and contribute to the management of inpatient rehabilitation beds where required. You will work closely with multidisciplinary teams including physiotherapists, nurses, discharge coordinators, social workers, and community services to ensure coordinated patient care and effective discharge planning.
