Link Worker

As a Link Worker, you will support and empower adults across Surrey to improve their mental health and wellbeing. You will provide person-centred one-to-one support, assessments, structured interventions and onward referrals, helping people identify their goals, make positive changes and connect with community activities and services. Location: Woking, supporting individuals across Guildford and Waverley, Surrey Heath and Farnham, and North West Surrey Salary: £26,040 Hours: 35 hours per week, full time Contract type: Not specified Key responsibilities: Support clients to identify their needs and goals and make positive changes to their mental wellbeing. Complete assessments, risk assessments, support plans, reviews and onward referrals where appropriate. Use the Recovery Star with clients, review progress regularly and record outcomes. Deliver motivational and structured psychosocial interventions that promote health and wellbeing. Maintain prompt, accurate and professional electronic and paper records in line with Catalyst policies. Connect clients with partner agencies, community organisations, groups and mainstream activities that meet their needs and reduce social isolation. Report incidents promptly and follow Catalyst policies and procedures. Work collaboratively with Group Workers and facilitate or support group activities where required.
