Leaving Care Worker A Local Authority is seeking a Personal Advisor to support young people leaving care and transitioning into independent adulthood. Key Responsibilities:Act as a named advisor for care leavers aged 18+Support young people aged 16-18 alongside Social Workers to prepare for independenceDeliver one-to-one and group interventions to develop life skills and improve education and employment outcomesComplete Needs Assessments and Pathway PlansProvide advice on housing, health, education, and employmentWork with multi-agency partners to coordinate supportUndertake visits, maintain accurate records, and manage caseloads Requirements:Level 3 qualification in a relevant field or substantial equivalent experienceExperience working with vulnerable young people or care leaversKnowledge of safeguarding and relevant legislationStrong communication and organisational skillsAbility to work independently and manage competing prioritiesAdditional:Flexible working, including occasional evenings and weekendsAbility to travel as part of the role ..... full job details .....
