This is a full-time role working 37 hours per week, Monday to Friday. Overview: The service follows a strong through care approach dedicated to supporting children and young people who have a permanence plan and who transition into becoming care experienced young people. The Aftercare Worker will support care leavers to achieve positive outcomes and successfully maintain independent living, while ensuring consistent support and guidance throughout their journey into adulthood.
