Learning Disability and Autism Manager An South London ICB 6 Month Contract A South London ICB is seeking an experienced Learning Disability and Autism Manager to support the delivery of its Learning Disability and Autism Programme, helping to reduce inappropriate hospital admissions and improve outcomes for children, young people and adults with Learning Disabilities and/or Autism. This is an initial 6-month contract, offering 37 hours per week with flexible hybrid working. Attendance on site is only required when needed. Key Details Initial 6-month contract An South London ICB 37 hours per week Flexible hybrid working On-site attendance only when required Rate negotiable Interviews taking place next week The Role You will play a key role in supporting individuals with Learning Disabilities and/or Autism to remain in their local communities and avoid unnecessary hospital admissions. Working closely with NHS providers, Local Authorities, Social Care, Education and wider MDTs, you will manage complex cases and support effective discharge planning. Key responsibilities include: Chairing and coordinating CTRs and CETRs Managing and overseeing Dynamic Support Registers Supporting admission avoidance and discharge pathways Working with community MDTs, Social Care and specialist LD services Identifying barriers to discharge and escalating risks Supporting individuals with complex Learning Disability and Autism needs Providing specialist advice and guidance to professionals and ..... full job details .....
