Lead Support Worker – Swindon, Wiltshire. Permanent, part time (22.5 hpw), working Monday to Friday between 9am and 5pm. Pattern to be discussed. Pay £13.21 per hour (£25,838 per annum) [pro rata] with benefits including Health Cash Plan.Home, a place where you belong. As our Lead Support Worker (known by us as Coordinator), you’ll support our customers who have complex needs to develop skills to live more independently in the community and help them achieve their hopes and aspirations.Responsibilities
Create support plans with customers and coordinate a small team of Support Workers working to the plans.Provide Mental Health support.Assist customers with daily living activities such as making meals, shopping, budgeting, maintaining tenancy, and moving to independent accommodation when able.Carry out risk assessments, support planning, goal setting and regular reviews.Engage with customers through regular interactions and collaborative team work; allow time for conversations and relationship-building.Contribute to a positive, supportive environment to help customers achieve positive outcomes.You bring
Passion to support customers to live their best life, with attention to detail and collaborative working.Experience creating person-centred support plans and guiding colleagues in executing the plans.Experience coordinating and assessing customer referrals.Ability to work on your own initiative, stay calm under pressure and maintain a resilient approach.Enhanced DBS check (we pay for this).Our team
