The RoleYou''ll work across four supported accommodation sites in Westminster, providing person-centred support that helps residents build confidence, maintain their accommodation, and move towards greater independence. Key Responsibilities Deliver one-to-one support and regular key work sessions Complete support plans, risk assessments, and reviews Promote recovery, wellbeing, and independent living Support residents to access healthcare, benefits, employment, training, and community services Work closely with mental health teams, probation, substance misuse services, and other partner agencies Maintain accurate records and contribute to positive outcomes for residents About You Experience supporting adults with ..... full job details .....
