Your responsibilities will include: Completing Care Act assessments and reviews. Managing a community caseload of adults with mental health needs. Supporting timely and effective discharge from inpatient mental health settings. Developing person-centred care and support plans. Coordinating appropriate care packages and community services. Identifying strengths, promoting independence and maximising people''s choice and control. Assessing and managing risk to ensure safe discharge arrangements.
