Role PurposeThe Pharmacy Technician will be a crucial part of the Clinical Pharmacy team within General Practice, aimed at enhancing patient care through effective medication management. This role supports the national policy directives, including the Five Year Forward View and GP Forward View, which emphasize the pivotal role of pharmacy in primary care.
Key ResponsibilitiesClinical Duties
Patient Interaction:
Engage in patient-facing roles to ensure the safe and effective use of medications through shared decision-making.
Medication Optimization:
Perform tasks like checking inhaler techniques, supporting medication reviews, and conducting medicines reconciliation.
Support Transitions:
Assist with medication reviews for new care home patients and coordinate medicine synchronization for patient transfers.
Structured Medication Reviews:
Organize necessary monitoring tests and support Clinical Pharmacists in conducting SMRs.
Public Health Contributions:
Offer lifestyle advice, service information, and tackle local health inequalities.
Protocol Management:
Manage shared care protocols and support antimicrobial stewardship initiatives.
Technical and Administrative Support
Process Efficiency:
Ensure efficient medicines optimization processes, including the implementation of the Electronic Prescription Service (EPS) and promotion of Electronic Repeat Dispensing (eRD).
Training and Support:
Provide training on the legal, safe, and secure handling of medicines.
Collaboration:
Develop relationships with pharmacy professionals and multidisciplinary team members to enhance care integration.
Policy Implementation:
Support the implementation of national prescribing policies and assist in delivering on QIPP, QOF, and locally commissioned enhanced services.
Quality Improvement:
Contribute to practice policies for CQC requirements and support quality improvement measures.
