Later moving to Monday-Sunday, 1 in 6 rotaAddress: 222 Upper Street, London, Islington, N1 1XREden Brown Synergy is currently recruiting for a qualified Occupational Therapist to join the Integrated Discharge Service (IDS) within Islington Adult Social Services.This is an exciting opportunity to join a dynamic and fast-paced Hospital Discharge Service, supporting Islington residents to return home safely while promoting independence, recovery and positive outcomes.The RoleAs an Occupational Therapist, you will play a key role in assessing risk and designing interventions to support residents at different stages of the hospital discharge pathway.Key responsibilities include:Supporting the Discharge to Assess pathway through referral screening and discharge planning.Participating in the 7-day screening rota, expected to commence after approximately 2 months.Completing post-discharge assessments and providing one-off therapy interventions.Assessing home environments and arranging equipment and minor adaptations.Providing advice on managing daily activities and manual handling.Developing Reablement Plans and Goals using a strengths-based, outcome-focused approach.Working closely with Social Workers, Reablement, Health colleagues and wider multidisciplinary teams.Supporting independence, recovery and ..... full job details .....
