This is a full-time role working 37.5 hours per week, Monday to Friday. Key Responsibilities Provide person-centred support to adults with multiple and complex needs. Manage a caseload and develop individual support plans. Support service users to identify and achieve personal goals. Help individuals develop the skills needed to live independently. Provide support with housing and tenancy-related issues. Assist with rent, rent arrears, bills, benefits and budgeting where required. Support individuals to maintain suitable accommodation and prevent homelessness. Help service users move towards permanent and sustainable housing. Provide advocacy and practical support where needed. Build positive and professional relationships with service users. Work with local authorities, housing providers and other agencies to coordinate appropriate support. Maintain accurate records, support plans and case notes.
