      <description>Location: Newport | Salary: 37.34-37.34 Hourly | Type: Permanent | Qualified Social Worker - Hospital Team St Woolos Hospital, Newport (Hybrid Working) £37.34 per hour Umbrella 37 hours per week Initial 3-month contract  Hoop Social Work is recruiting for an experienced Qualified Social Worker to join the Hospital Team based at St Woolos Hospital in Newport. This is a fantastic opportunity to work within a supportive, fast-paced multidisciplinary team, with a hybrid working arrangement combining time on-site and working from home. As a Hospital Social Worker, you will play a key role in supporting timely, safe hospital discharges by completing strength-based assessments and coordinating appropriate care and support plans. You will work closely with health professionals, patients and families to achieve positive outcomes while promoting independence. Key Responsibilities   Complete assessments in accordance with the Social Services and Well-being (Wales) Act.   Undertake hospital discharge planning in a busy acute setting.
