The London Borough of Lambeth are recruiting for an adult Social Worker to join their Hospital Team at St Thomas'' Hospital. This role is for a full-time locum and offers hybrid working. The Role: Hospital Social Worker post available in the Hospital Team based at St Thomas'' Hospital - to start ASAP! 5 day working - Monday - Friday 35 hours per week 3 days in the office About the Team: This team supports Lambeth residents who are admitted to Guy''s and St Thomas'' Hospital (GSTT) and hospitals out of the borough. The primary feature of work undertaken by staff in the Hospital team is to deliver strength based preventative support, where require, assess, and establish supports and safeguard patients. The team undertake reviews of existing packages of care and consideration of placements. Hospital social workers carry out a variety of responsibilities, including referring people to reablement where they are identified as potentially benefitting from maximising their everyday living skills, ensuring safe and timely decisions are made to facilitate discharges. The team work closely alongside our MDT colleagues and voluntary sector organisations. The Hospital is a fast paced team with a committed and enthused workforce who strive to achieve positive outcomes for our residents.
