Hospital Social Worker

Eden Brown Synergy Recruitment are currently recruiting for an experienced Hospital Social Worker to join a busy and supportive Adult Social Care team in Newham. This is an excellent opportunity for a skilled practitioner looking to work within a fast-paced hospital setting, supporting timely and safe discharges while making a real difference to the lives of local residents.

Working as part of a multidisciplinary team, you will play a key role in facilitating effective discharge planning, undertaking Care Act assessments, mental capacity assessments, safeguarding enquiries, and coordinating support for adults with complex needs.

About You:

To be considered for this role, you must have:

Social Work England registration
A recognised Social Work qualification (Degree, DipSW or equivalent)
A minimum of 3 years post-qualified experience
Recent experience within a Hospital Social Work or Adult Social Care setting
Sound knowledge of the Care Act, Mental Capacity Act, safeguarding procedures and hospital discharge pathways
Excellent assessment, communication and partnership working skills
