Hospital Social Worker

Eden Brown Synergy Recruitment are currently recruiting for an experienced Hospital Social Worker to join a busy and supportive Adult Social Care team in Newham. This is an excellent opportunity for a skilled practitioner looking to work within a fast-paced hospital setting, supporting timely and safe discharges while making a real difference to the lives of local residents. Working as part of a multidisciplinary team, you will play a key role in facilitating effective discharge planning, undertaking Care Act assessments, mental capacity assessments, safeguarding enquiries, and coordinating support for adults with complex needs. About You: To be considered for this role, you must have: Social Work England registration A recognised Social Work qualification (Degree, DipSW or equivalent) A minimum of 3 years post-qualified experience Recent experience within a Hospital Social Work or Adult Social Care setting Sound knowledge of the Care Act, Mental Capacity Act, safeguarding procedures and hospital discharge pathways Excellent assessment, communication and partnership working skills Whats on offer: Competitive pay rate of 32 per hour Flexible locum contract Supportive and collaborative team environment Opportunity to work alongside health professionals in a multidisciplinary setting Valuable experience within one of London''s most diverse and dynamic boroughs Regular supervision and management support Why Newham?
