OverviewGrade 6 Days: Monday - Friday Shifts between: 08:00 - 22:00Shift patterns: 08:00 - 16:30 or 10:00 - 18:30Pay rate: £17.18 per hour1 hour paid lunch We are seeking a dedicated Support Assessor to join our team. This role involves assessing, developing, and contributing to the monitoring and review of individual reablement programmes within the provider service. The aim is to meet the identified needs of service users, including those with complex and changing needs, to promote independence, choice, control, and social inclusion.Day-to-Day Responsibilities:Carry out service-specific assessments to identify individual needs within the provider service remit.Develop individual programmes to meet the needs of all service users, including those with complex and changing needs.Plan and organise work to meet personal and service targets and deadlines.Work in partnership with other team members, services, and organisations as required.Monitor and review individual programmes with service users, carers, staff, and other professionals using agreed systems of communication.Follow appropriate systems, processes, and communications to ensure services are set up and delivered on time.Record user information, maintain and update files, and prepare and produce reports.Participate in training, supervision, and appraisal to ensure the delivery of a quality service.Attend staff, user, and carer consultation meetings to assist in the development and planning of future services.Work ..... full job details .....
