The Role You will work directly with people experiencing homelessness or at risk of homelessness, providing practical support, advice and guidance to help individuals access appropriate accommodation and support services. Key Responsibilities Carry out street-based outreach and engage with people experiencing homelessness. Build positive, trusting relationships with service users. Assess individual needs and identify appropriate support pathways. Provide advice and assistance around housing, benefits, health and other support needs. Liaise with local authorities, housing providers and partner agencies. Support individuals to access accommodation and relevant services. Maintain accurate case notes and records.
