Home Care Worker

• Drive clients to appointments, errands, or social activities using your own vehicle or company vehicle
• Maintain accurate records of care provided, including progress notes and any changes in condition
• Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery
• Monitor client well-being regularly, reporting any concerns or incidents promptly

Qualifications

• Valid driving license with reliable access to a vehicle for client transportation
• Previous experience working in care homes or providing senior care is highly desirable
• Supervising experience or leadership skills to oversee client activities and assist other team members when needed
• Knowledge of creating and implementing care plans tailored to individual needs
• Excellent communication skills for building trusting relationships with clients and their families
• Basic IT skills for maintaining accurate documentation and updating electronic records
