OverviewJob summary: This is an opportunity to work in the Planned Therapy Team - Sefton Place on a part-time fixed-term basis. The service provides Physiotherapy, Occupational Therapy, and falls management to a diverse population. The post holder will participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services, promoting effective teamwork, and delegating activities appropriately to benefit patients and service delivery. The role will involve hands-on clinical practice, education and training, and research. The post holder will manage a specialist and complex caseload of patients with co-morbidities as an autonomous practitioner, using evidence-based, patient-centred principles to assess, plan, implement and evaluate interventions. The post holder will plan, develop and evaluate Physiotherapy Services within a designated area/team, taking a holistic approach to assessment and treatment and providing high-quality care to a defined, complex caseload using appropriate models of care and evidence-based practice. Postholders will work with other professionals and agencies to enable patients to be maintained in an appropriate environment.

Main duties of the job

Provide expert clinical leadership within your area of specialism and implement best practice evidence-based interventions.

Undertake timely, holistic assessments of patients, including functional and environmental factors.

Demonstrate knowledge and experience in delivery and interpretation of outcome measures and other specific assessment tools.

Undertake autonomous assessments and work with other MDT members.

Provide comprehensive assessments and interventions using education and treatment facilities where appropriate.

Make rapid decisions using clinical reasoning, with appropriate support.

Collaborate with clinical colleagues and teams to deliver an optimal service within your specialism.

Attend relevant meetings linked to the service when appropriate.

Manage a clinical caseload within multidisciplinary teams, ensuring patients and carers receive optimal therapy and holistic care within available resources; monitor progress and adapt plans using outcome measures and reflective practice.

Keep accurate, up-to-date records on patient care and equipment provision in line with Trust/Professional/Service policies and legal requirements, and share as appropriate with colleagues and carers.

Person SpecificationQualifications – Essential

Diploma/Degree in Physiotherapy with current registration with CSP and HCPC as a Physiotherapist.

Evidence of CPD relevant to treatment of older adults, physical disability and rehabilitation.

Experience and/or additional education within your specialism; up-to-date CPD portfolio.

Qualifications – Desirable

Membership of the Chartered Society of Physiotherapy (CSP).

Postgraduate qualification or Masters level study related to your specialism; management/leadership training.

Membership of CSP.

Knowledge/Experience – Essential

Recent post-graduate experience in your specialism.

Experience in community or primary care settings; multidisciplinary/interdisciplinary work.

Experience with complex needs and delivery of complex care plans; coordinating and managing a team; supervision and teaching; service development.

Knowledge/Experience – Desirable

Experience in multi-agency working; promoting services within a locality; virtual ward experience in a community setting (as relevant).
