Highly Specialist Pathway 3 Occupational Therapist/ Physiotherapist. This post is part of Westminster CIS (Tri-borough service in Hammersmith and Fulham, Kensington and Chelsea and Westminster) and is based across Ellesmere and Forrester Court Care Homes with the office base at Hathaway House, W9. The role is suitable for a band 7 or an experienced band 6 looking to work in a dynamic and fast-moving community rehab service.An exciting opportunity has arisen for a highly specialist OT/PT to be part of our Rehabilitation team. You will provide decisive and effective therapy input, demonstrate leadership, have excellent communication skills, and take a flexible approach when working under pressure. You will be passionate about the value of community rehabilitation and preventing hospital admissions.Responsibilities
Complete complex assessments and interventions and make high-impact clinical decisions.Work autonomously while drawing on support from colleagues in primary and secondary care.Use evidence-based principles and specialist skills to assess, plan, implement and evaluate interventions, ensuring team-wide adherence to best practice.Lead on supervision and guidance to junior therapists and therapy students to deliver evidence-based rehabilitation.Manage your own caseload, providing home and care home visits and assessments.Lead on manual handling, pressure care and equipment needs within the team and support the team lead; deputise for the Therapy lead as appropriate.Pathway and Setting
The caseload is split between community work and Pathway 3, including step-down patients within nursing homes, with MDT support. The role involves working with adults with complex health needs and opportunities to develop a wide range of clinical skills (e.g., discharge planning, falls and frailty, orthopaedics, general medical and surgical conditions). An understanding of multi-pathology management and the impact of psychological/cognitive factors is desirable.Location and Details

HCPC registrationDegree/Diploma in Occupational Therapy or PhysiotherapyEvidence of continued professional developmentExtensive post-qualification experience in NHS or similar healthcare economySubstantial rehabilitation experience and ability to manage a complex individual and team caseloadExperience supervising junior colleagues and students; MDT workingDesirable Qualifications and Experience
Post-graduate qualification relevant to healthcareKnowledge of long-term conditions including end of life care and NHS modernisation agendaPrevious community therapist experience and experience leading a teamExperience in case management including multi-agency working; performance management and dealing with complaintsOther Information
