As a Health and Wellbeing Practitioner, you''ll play a key role in supporting individuals in prison to improve their health and wellbeing, reduce harm and prepare for a successful transition back into the community.What You''ll Be DoingManage your own caseload, building positive and supportive relationships with service users.Provide appropriate harm reduction advice, guidance and support.Complete comprehensive assessments and formulate individualised care plans.Work collaboratively with service users to ensure true co-production and that their goals and needs remain at the heart of their care.Deliver structured 1:1 interventions and support sessions.Facilitate engaging and effective group-work sessions.Support individuals with release planning, helping them prepare for life beyond prison.Coordinate effective through-care and transitional support, ensuring individuals are connected with the right services when returning to the community.Maintain accurate, timely and high-quality records, ensuring data is captured effectively.Work towards agreed targets and outcomes, using data to demonstrate the impact and effectiveness of your work.What We''re Looking ForWe''re looking for someone who:Is passionate about ..... full job details .....
