Health and Wellbeing Practitioner- HMP Send (part time)Location: SurreySalary: £13,928.57 per annumVacancy Type: Permanent, Part TimeAbout The RoleThe Forward Trust is seeking a compassionate and motivated Health and Wellbeing Practitioner to join our team at HMP Send, Surrey, working 18.75 hours per week. As part of our integrated Health and Wellbeing Service, you''ll support women experiencing substance misuse and low-level mental health challenges, helping them build healthier, more positive futures within a trauma-informed and gender-responsive environment.Working as part of a multi-disciplinary team, you''ll deliver recovery-focused interventions, provide practical support, and help service users prepare for successful reintegration into the community.What you''ll be doing:Managing a caseload and coordinating individual support plansConducting assessments and developing person-centred care plansDelivering structured one-to-one sessions and facilitating group programmesProviding harm reduction advice and guidanceSupporting release planning and through-care arrangementsWorking closely with healthcare, mental health, and prison colleaguesMaintaining accurate records and contributing to service performance targetsUndertaking second signatory duties and oral swab testing where requiredWhat we''re looking for:Experience supporting individuals with substance misuse, mental health, or complex needsExcellent communication and relationship-building skillsA proactive, organised, ..... full job details .....
