Health and Wellbeing Practitioner Location: Cardiff Salary: £26,787 per annum Vacancy Type: Permanent About The Role We''re looking for a passionate and proactive Health and Wellbeing Practitioner to join our team at HMP Cardiff on a full-time basis (37.5 hours per week). This is an opportunity to support individuals on their journey towards positive change, helping them improve their health, wellbeing, and prospects for a successful return to the community. You''ll manage a varied caseload, deliver tailored interventions, and work collaboratively with service users to develop meaningful care and release plans that support long-term recovery and wellbeing. What You''ll Be Doing Managing a caseload of service users and coordinating their treatment journeys Conducting assessments and developing person-centred care plans Delivering structured 1:1 interventions and facilitating group sessions Providing harm reduction advice and wellbeing support Supporting release planning and through-care arrangements for individuals returning to the community Carrying out second signatory duties and supporting clinical services Working closely with multidisciplinary teams and partner agencies Maintaining accurate case records and performance data to meet service targets What We''re Looking For Experience supporting individuals with complex health, wellbeing, substance use, or related needs Strong assessment, case management, and communication skills Ability to build positive relationships ..... full job details .....
