The Forward Trust is looking for a Health and Wellbeing Practitioner to join our team at HMP Highdown, working full-time, 37.5 hours per week. You ll play a key role in delivering recovery-focused support to people in prison, working as part of a skilled, multi-disciplinary team to provide psychosocial and clinical interventions using a trauma-informed and gender-responsive approach. What you ll be doing Manage a caseload, prioritising individuals according to their needs and release dates. Complete assessments, risk assessments and care plans, working collaboratively with service users.
