Health and Housing Coordinator

Role Summary We are looking for a compassionate and proactive individual to join the Integrated Discharge Team at Maidstone Hospital. In this role, you will help ensure patients can leave hospital safely and promptly by identifying and resolving barriers to discharge. Working closely with patients, families, healthcare professionals, and community services, you will support individuals transitioning from hospital back into their homes and local communities. The role primarily supports patients at Maidstone Hospital, with occasional work at Tunbridge Wells Hospital, cottage hospitals, and step-down bed facilities. Home visits will also form part of your responsibilities to assess discharge needs and provide follow-up support.
