As Head of Health and Social Care Integration, you will play a key role in developing and overseeing the delivery of a sustainable change programme which will improve the health and wellbeing of Wokingham residents. This programme of work will contain supporting the delivery of common schemes across the Integrated Care Partnership in Berkshire West and the delivery of local schemes, including the Better Care Fund, to meet local needs and reduce health inequalities in accordance with the council''s ambition of becoming a ''Marmot Borough''.
