Southwark Council are looking for an Head of Age Well for managing NHS Continuing Health Care (CHC) team and Better Care Fund (BCF) initiatives including developing proposals for BCF and reporting the NHS England. 425 per day - Please note the rate for this role is set my NHS Agenda for Change and is not negotiable at 425 per day Umbrella Pay Rate.Hybrid Working usually 2-3 days in the office but expected to be flexible for external meetings.This post is joint funded by the NHS, it is essential that candidates have experience of working in the NHS or an integrated commissioning function. Job Description:This role is a key part of the commissioning team which is funded by Southwark Council and SEL ICS.
