This is an exciting opportunity to work within a dedicated review team, helping to deliver innovative care solutions that promote independence and improve outcomes for service users.   Key Responsibilities    Deliver specialist occupational therapy services as part of the Double Handed Care Project.  Carry out comprehensive assessments and develop person-centred care plans.  Provide expert moving and handling assessments and recommendations.  Train and support care staff to ensure safe and effective implementation of care plans.  Review and adapt care plans in line with best practice and service user needs.  Maintain accurate case records and undertake community visits as required.
