Thepost holder will work as part of a team for Primary Care Enhanced AccessService based in
both
Havant and Waterlooville.Thepost holder will provide clinical assessment and treatment for the deliveryof a safe, efficient, and reliable clinical service. They will work closely with the ClinicalLead, and Service Manager. They willform links and develop relationships with secondary care and GP Servicecolleagues and those working in the wider healthcare community. The postholder will ensure patients receive optimal care in the correct settingwithin the healthcare system. Healthpolicies and standard operating procedures will be adhered to and, ifrelevant, refinedIn theplanned primary care clinic, the patient will undergo clinical assessment andreview, examination, appropriate investigations requested e.g., x-rays,scans, blood tests; and a diagnosis and management plan will be agreed. Clinical DutiesThe GP will work with a multi-disciplinary team to ensure appropriatecare is delivered in the right place, at the right time. The post holder will be expected to:Maintainthe highest clinical standards for all patients accessing the service.Assess,investigate, diagnose, and treat patients presenting with clinical needswhich can be met by primary care ensure appropriate coding planned andopportunistically to support the delivery of QOF, LCS, DES and CQUINs.Referto secondary care where appropriate using the appropriate referral forms andmechanisms.Ensurerequesting of appropriate investigations and reporting on these whenrequired.Provideemergency care while waiting for emergency transfer of patient whenappropriate.Provideadvice and support to colleagues and the multi-disciplinary team.Communication and WorkingRelationshipsGPTeam including Clinical Team of Nurses and Paramedics, Operational Manager,and non-clinical team.GPswithin Primary CareSecondaryCare e.g., radiology and pathologyWiderhealthcare servicesThe post holder will be expected tocontribute to the development of the service.It is expected that the post holder will:Supportto improve non-elective care pathways within the healthcare community.Communicatewith the Governance Lead and support the delivery of a high-quality serviceand raise any governance issues that may arise.Supportthe teaching and development of doctors, nurses, and professionals from otherdisciplines such as Emergency Care Practitioners, GP registrars in PrimaryCare Skills. This may involvepractitioners sitting in and seeing patients where appropriate.Workwith the Service Manager to ensure training, significant events, complaints,audits, surveys, and other professional service development activities areachieved to ensure the smooth running of the service.Maintaincontinued education through attendance at courses and/or study days asnecessary to ensure professional development requirements for on-goingprofessional registration and licensing are met.Participatein, encourage, and support professional development within the service.Ensurethat high clinical standards are maintained throughout the service.Participatein annual appraisal and complete an annual personal development plan.Complywith standard operational procedures and departmental and organisationalpolicy.Undertakenecessary mandatory training.Strategic Development,Planning and OrganisingActivelyparticipate in both departmental and organisational matters concerningclinical governance, care pathways, team meetings and audit when required.Person Specification
Experience
An experienced doctor with GP accreditationCurrent registration with GMC and NHS performers list (with no conditions preventing working or restrictions on practice)Experience of working in primary care and urgent care environmentRecord of on-going CPD and learningMRCGP examination or equivalent experienceSound knowledge of QOF, LCS, DES, CQUINs and use of appropriate codingWill have an understanding of the political agenda and drivers influencing primary care provision.Good leadership skills with the ability to influence and negotiate.Good organisational and IT skills desirableExcellent communication and interpersonal skills written, verbal (Particularly listening skills)Clear understanding of professional responsibility and accountability.Good organisational skills and ability to manage and prioritise own workload.Good clinical skills in assessment, diagnosis, and management of acute Primary care presentationsBe able to clear an enhanced DBS check.Must be authorised to work in the EEC.Excellent communication and interpersonal skills written, verbal (Particularly listening skills)Clear understanding of professional responsibility and accountability.Good organisational skills and ability to manage and prioritise own workload.Good clinical skills in assessment, diagnosis, and management of acute Primary care presentationsBe able to clear an enhanced DBS check.Ideally have knowledge of EMIS Clinical IT systemDisclosure and Barring Service Check
