We are recruiting for a Frailty EAAR (Early Alternative to Admission Review) Clinical Nurse Specialist who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.We are a multidisciplinary team comprising Consultant Geriatricians, Advanced Clinical Practitioners (ACPs), Clinical Nurse Specialists, and a Frailty Lead Pharmacist. Our team operates a seven-day service based in the Emergency Department (ED) and Acute Medical Unit (AMU), providing specialist assessment and care for frail and vulnerable patients.As part of the Integrated Care Board''s (ICB) initiative to reduce unplanned hospital admissions by 25%, one of the key interventions within the acute trust is the Early Alternative to Admission Review (EAAR), conducted upon patients'' arrival to the hospital.This role involves clinically assessing frail patients who present to the Emergency Department, serving as a secondary triage to identify and direct suitable patients onto alternative care pathways at the point of arrival.For patients not appropriate for the EAAR pathway, the focus will be to initiate a comprehensive geriatric assessment (CGA) aimed at minimising duplication, reducing time spent in the ED, and improving patient outcomessupporting a "home first" model of care.Main values: Include, Respect, and Improve. The post involves planning, implementing, and evaluating care independently as an autonomous practitioner with specialist skills, encompassing a broad range of clinical responsibilities.The Clinical Nurse Specialist will be responsible for identifying and assessing patients'' suitability for the EAAR pathway, working closely with the Care Coordination Hub to ensure patients are directed to the most appropriate care options.The successful candidate will need to confidently triage patients using a variety of assessment tools and advanced clinical skills to support timely and accurate clinical decision-making.In collaboration with the frailty consultant, the post holder will determine whether patients meet the criteria for discharge without further assessment and facilitate their transition to suitable care pathways via the Care Communication Hub.About us
