This is a varied role where you''ll provide additional support across multiple services and locations, stepping in where extra capacity is needed. Working across supported housing and community outreach services, you''ll support people to build independence, improve wellbeing and access the services they need. No two weeks will look the same, making this an ideal opportunity for someone who enjoys variety, can adapt quickly, and is passionate about delivering person-centred, trauma-informed support.Day-to-Day Duties:Provide practical and emotional support to individuals with a range of complex needs.Build positive, professional relationships that promote independence and recovery.Deliver person-centred, strengths-based and trauma-informed support.Offer advice, guidance and signposting to appropriate services and community resources.Maintain accurate and up-to-date client records and support plans.Work ..... full job details .....
