The service works in close partnership with Wandsworth Adult Social Care, housing providers, primary care, and local VCSE partners to deliver holistic, multi-agency reablement pathways. This is an exciting opportunity to help adults experiencing mental health challenges build independence, sustain their tenancies, and achieve positive outcomes through person-centred, trauma-informed support. As the operational lead, you will coordinate day-to-day service delivery, oversee referral and triage processes, support and supervise staff, and work closely with partners across health, housing, and social care services.
