The Move On and Activities Support Worker for South Manchester mental health services primarily based in our Lone parent service in Wythenshawe, plays a key role in supporting people to move on successfully from supported or temporary accommodation into sustainable, independent housing, while promoting wellbeing, independence and meaningful community connection. The role is strongly focused on active move on work, supporting individuals and households to explore all appropriate housing options, secure tenancies, and develop the skills and confidence that they have gained in the supported living setting and use this into more independence in the community . some transition support to enable people to settle successfully and sustain their homes may be beneficial to some tenants and families moving on.
