Working as part of the Family Hub model, you will provide early intervention and practical support to help improve outcomes for families, ensuring children are safe, supported and able to thrive. Key Responsibilities Provide early help support to children, young people and families using a whole-family approach. Manage a caseload as a Key Worker, coordinating assessments, support plans and interventions. Work closely with partner agencies including schools, health professionals, early years providers, housing and police. Deliver one-to-one and group programmes for children, young people and parents.
