This specialist, recovery-focused service supports people in their own homes following illness, hospital discharge or changes in health. Unlike long-term care, this is a time-limited service designed to help individuals regain independence, rebuild confidence and achieve personal goals. If you are passionate about empowering people rather than doing everything for them, this role offers meaningful, outcome-driven work in the community. About the Service Our Short-Term Homecare Team provides temporary support to help people: Remain safely at home during periods of increased difficulty Recover after a fall, illness or operation Regain confidence following hospital discharge Live well with progressive illness, including end-of-life support You will actively support individuals to rebuild daily living skills, increasing independence as they progress.
