This is an immediate agency opportunity for a highly organised SEND professional with strong knowledge of Education, Health and Care Plans and the annual review process.The RoleAs an EHCP Coordinator, you will manage an allocated caseload of post 16 students with EHCPs and High Needs funding. You will coordinate and chair annual reviews, gather contributions from families and professionals, and ensure documentation is completed accurately within statutory timescales.The EHCP Coordinator will work closely with learning support teams, curriculum staff, parents, local authorities and external agencies to ensure students receive the provision outlined within their plans.Key ResponsibilitiesCoordinating and chairing EHCP annual review meetingsManaging annual review documentation and statutory deadlinesGathering feedback from students, parents, staff and external professionalsMonitoring whether agreed SEND provision is being deliveredSupporting students transitioning into and through collegeLiaising with local authorities regarding consultations and High Needs fundingMaintaining accurate student records and management informationProviding ..... full job details .....
