As our services continue to grow, we are looking for an organised, compassionate and proactive Care Coordinator to join our team and play an important role in the successful delivery of our care services.
The Role
As a Care Coordinator, you will be responsible for supporting the smooth and effective day-to-day coordination of our domiciliary care services.
Working closely with service users, their families and our care team, you will coordinate staff rotas, manage changes to care arrangements and help ensure that every service user receives safe, consistent and high-quality care.
This is a fast-paced, office-based position that would suit someone who is highly organised, confident communicating with others and able to remain calm and solution-focused when dealing with unexpected situations.
Key Responsibilities

Planning, creating and maintaining staff rotas using care scheduling systems

Matching care workers to service users according to their individual needs, availability and geographical location

Managing staff absences and arranging appropriate cover at short notice

Maintaining regular communication with service users, families and care workers

Monitoring care delivery and addressing any issues promptly

Updating care plans, schedules and service user records accurately

Supporting the Care Manager with compliance, quality assurance and CQC requirements

Carrying out spot checks, staff supervisions and field-based support when required

Managing incoming calls, enquiries and concerns in a professional and compassionate manner

Identifying and resolving scheduling and care delivery issues efficiently

Maintaining continuity and consistency of care for all service users

Supporting the wider team with the day-to-day running of the service
Skills & Experience
Essential:
-
Previous experience working within a domiciliary care environment
-
Good understanding of care standards and CQC requirements
-
Excellent organisational and time-management skills
-
Ability to prioritise tasks and work effectively under pressure
-
Strong problem-solving and decision-making skills
-
Excellent communication and interpersonal skills
-
Good IT skills and confidence using care management or scheduling systems
Desirable:
-
Previous experience in a Care Coordinator, Scheduler or similar role

Experience using electronic care planning or rostering systems
-
Full UK driving licence
