This is an exciting opportunity to support the delivery of responsive, high-quality end of life care in the community, helping patients remain in their preferred place of care and avoid unnecessary hospital admission.    ABOUT THE ROLE  	 Support the Clinical Lead in the operational management and day-to-day delivery of the EOLUCS team. 	 Provide clinical leadership, specialist assessment and decision-making for patients receiving end of life care in the community. 	 Help coordinate workloads, support team development and contribute to service improvement.
