Dementia Navigator

At Alzheimer''s Society, we believe no one should face dementia alone. Every conversation, every visit and every small moment of reassurance can make a real difference. We are looking for a warm, thoughtful and organised Dementia Navigator to join our supportive team in Bristol working 21 hours per week. This is a community-based role, so you''ll regularly travel across Bristol area to meet people in their homes, community venues and partner locations. As such, the ability to travel independently across the area is essential. As a Dementia Navigator, you''ll take a person-centred approach to help people understand their diagnosis, navigate health and social care services, access local support and feel more in control of their lives. You''ll build trusted relationships with individuals, families and professionals, ensuring people receive the right support at the right time.You will be part of a pro-active, caring and supportive local team who work in a fully integrated partnership with NHS colleagues, supporting people from point of diagnosis through to end of life.
