The Role: Dementia Care Manager Our Dementia Care Managers lead on the care and management of our Dementia Community with the home. They deliver and provide all the dementia related learning to the rest of the team and provide support and knowledge to the residents and their families. Key Responsibilities: Residential Care- Understands and promotes the Hallmark Dementia Strategy while working alongside the team to ensure relationship centred care is delivered to a high standard. Plans, develops and organises the overall operation of the Dementia Community. Compliance- Developed a working knowledge of current and evolving laws of policies and practice while ensuring the dementia community meets all CQC standards and regulations. ..... full job details .....
