Every conversation, every visit and every moment of reassurance can make a real difference to someone living with dementia and the people who care about them. We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive Worcestershire team, working 28 hours per week across North/Central/South/West on a permanent basis. We''re launching a new county-wide dementia support service across Worcestershire in partnership with Herefordshire and Worcestershire Health and Care NHS Trust, and we''re looking for Dementia Advisers to join us at the start of this exciting journey. This is a community based role. You will spend much of your time out in local communities, working in GP Practices meeting people face to face, carrying out home visits, attending community groups and building relationships with local organisations. While some administrative work can be completed from home, this role is centred on being present in the community and supporting people where they are. As a Dementia Adviser, you''ll support people from the point of diagnosis and throughout their dementia journey. You''ll take time to understand what matters to each person, provide information and guidance, and help them access the support they need to live as well as possible.
