About the opportunityAt Alzheimer''s Society, we believe no one should face dementia alone. Every conversation, every visit and every small moment of reassurance can make a real difference. We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive team in City of Doncaster working 35 hours hours per week.The Doncaster service is a partnership with Rotherham, Doncaster and South Humber NHS Foundation Trust to provide joined-up support for people affected by memory concerns and dementia. This includes both practical advice and access to clinical expertise through a shared pathway.As part of a supportive multi-disciplinary team, you''ll play an important role in helping people with dementia and their carers achieve better outcomes and feel more supported in their day-to-day lives.You''ll also get out into the community to raise awareness of the service, helping people understand what support is available and how to access it. Working alongside clinical colleagues, you''ll build strong relationships with primary care teams so we can reach those who may not yet be receiving the help they need.This is a busy and rewarding service, so being flexible and able to manage different priorities is important.
