Duties and Responsibilities: * Manage a caseload of Children in Care (CIC) and Looked After Children (LAC) aged 16+. * Undertake statutory visits, assessments, care planning, and pathway planning to support young people preparing for adulthood. * Build effective relationships with young people, carers, families, and partner agencies to achieve positive outcomes. * Support young people with education, employment, training, accommodation, health, and independence planning. * Prepare high-quality assessments, reports, care plans, and court documentation where required. * Participate in statutory reviews and ensure all case records are maintained accurately and within timescales. * Ensure all work is completed in accordance with relevant legislation, policies, and best practice guidance.
